Nomination Form

This is our first Awards Evening and this is the ideal opportunity to give pupils the recognition they deserve for their hard work, commitment and passion for Physical Education and School Sport. 

The Awards Evening will take place on Tuesday 7 July at Cottingham High School.  Nominations can be made by completing the nomination form and returning it to Rebecca D'Arcy, Partnership Development Manager.  The closing date for entries is 8 May 2015.
